I can’t find a fulltext copy of citation 41, it’s from 1996.
Citation 42 is from 1984
also not available on the public internet.

I’ve got a copy of the JAMA 1984 study now.

The letter from 1996 to the BJA doesn’t seem to contain the 0.1% to 0.25% incidence rate.
